Your health and safety is our number one priority.

We understand the severity of COVID-19, the importance of social distancing and enforcement of health department guidelines. For your safety we have implemented additional infection prevention and control measures to our current workplace protocols in order to provide you with a safe environment. If you experience any COVID-19 symptoms, please let us know as soon as possible so we can defer your appointment.

Please note the following:

1. All visits are by appointment only. We are limiting the number of clients in our studio in order to maintain appropriate social distancing. Please arrive to your appointment alone. Family members/ friends/children will not be permitted in the studio.

2. When arriving to your appointment you will be asked to call/sms the studio prior to arriving so to prevent you and the previous client from being in the studio at the same time.
3. All clients will be required to wear a mask and sanatize hands upon arrival to appointment. Due to the nature of the service and proximity between tech/client, it is imperative clients follow this protocol. Please be advised, if this is not followed we will need to reschedule your appointment.

We sanitize our facility thoroughly before and after every client, this includes disinfecting all surfaces, equipment, and seating areas. We are committed to serving you in an efficient and safe way, so please follow all guidelines.

ALTHOUGH WE TAKE GREAT CARE TO BE MASKED AND RETAIN HIGH HYGENIC STANDARDS, WE ARE NOT ABLE TO ENSURE CLIENTS WILL NOT BE EXPOSED TO COVID-19, GIVEN THE CONTAGIOUS NATURE OF THIS VIRUS, WHEN COMING IN FOR THEIR APPOINTMENTS.

BY COMING INTO THE SPA FOR YOUR APPOINTMENT YOU RELEASE ALL STAFF AND SPACE OF LIABILITY.

I understand that I am opting for an elective treatment/procedure/service that is not urgent or necessary and is a service solely by my personal choice. I also understand that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. I further understand that COVID-19 is extremely contagious and is believed to spread by person-to-person contact; and, as a result, federal and worldwide health agencies recommend social distancing for the immediate and foreseeable future. I recognize that Violetta Bontes-Forward (Invisible Ink by Vita) are closely monitoring this situation and have put in place reasonable preventative measures aimed to reduce the spread of COVID-19 in order to provide a safer environment for their clients. Given the nature of the COVID-19 virus, I understand there is an inherent risk of becoming infected with COVID-19 by virtue of proceeding with this elective treatment/procedure/service. I hereby acknowledge and assume the risk of becoming infected with COVID-19 through this elective treatment/procedure/service, and I give my permission for Violetta Bontes-Forward (Invisible Ink by Vita) to proceed with my service. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any infection or injury to myself, including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with Violetta Bontes-Forward, Invisible Ink by Vita, Liron Bartman or Beauty Brows by Li. I understand that possible exposure to COVID-19 before/during/after my elective treatment/procedure/service may result in the following: a positive COVID-19 diagnosis, extended quarantine/self-isolation, additional tests, hospitalization that may require medical therapy, Intensive Care treatment, and possible need for intubation/ventilator support, short-term or long-term intubation, other potential complications, and the risk of death. I understand that COVID-19 may cause additional risks, some or many of which may not currently be known at this time, in addition to the risks described herein, as well as those risks for the treatment/procedure/service itself. I have been given the option to defer my treatment/procedure/service to a later date. However, I understand all the potential risks, including but not limited to the potential short-term and long-term health and financial complications related to COVID-19, and I would like to proceed with my desired/elective treatment/procedure/service. I CONFIRM THAT AT THE TIME OF SIGNING, I AM NOT COVID-19 POSITIVE, I HAVE NOT BEEN EXPOSED TO SOMEONE WHO IS COVID-19 POSITIVE AND DO NOT HAVE ANY COVID-19 SYMPTOMS. I UNDERSTAND THE EXPLANATION, MY QUESTIONS HAVE BEEN ANSWERED AND I CONSENT TO THE PROCEDURE KNOWING THE RISKS INVOLVED.

When coming to your appointment, please be sure to be wearing a face mask. I will have hand sanitizer and a place to wash your hands upon your arrival.
